Register as a New Patient

If you would like to join the practice you should ideally bring proof of address (Utility Bill etc) and your NHS (National Health Service) number with you when you come to register. You will then be given a registration form and Patient Questionnaire to complete. After this is returned to the practice you will be given an appointment to see the Health Care Assistant for a New Patient Check, and this would complete the registration process.

Children 16 and under do not need to complete the registration form. An adult member of the family can register them by providing the required information and signing the registration application on their behalf.

If you wish to pre-register click on the links below to open the forms. When you have completed all of the details, click on the "Submit" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Named GP

As part of the National GP contract, all patients now have a ‘named’ GP.

The patients at the Strand Medical Centre are registered with Dr A Samaei and the Family Practice patients in Middleton are registered with Dr Khairuddin Mat Lazin

You probably think that you already have a named GP or a GP whom you would normally visit at the surgery. This WILL NOT affect your choice of doctor.
If you see a particular doctor at the surgery, you should still continue to see him/her.

The ‘named’ GP will, however: –

  • Take lead responsibility for ensuring that all appropriate services required under the contract with the practice are delivered to you.
  • Where required, based on the professional judgement of the ‘named’ GP, work with relevant associated health and social care professionals to deliver a multidisciplinary care package that meets your needs.
  • Ensure that your physical and psychological needs are recognised and responded to by the relevant clinicians in the practice.
  • Ensure that you have access to a health check if requested, which is already a requirement of the GP contract regulations.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
